Nomination for Ohio State USBC Hall of Fame
Please click on the following links to download the forms for nominating individuals for the Ohio Bowling Hall of Fame. There are three categories to choose from. Below is brief explanation of each category:
Award for Meritorious Service
This award is for anyone contributing outstanding service to the general welfare and progress of the game of bowling in the State of Ohio for a period of fifteen (15) years. (Requirements for years of service do not apply for posthumous candidates.)
Award as a Pioneer
This award is for anyone who has contributed outstanding service and/or have sanctioned/certified bowling accomplishments worthy of Superior Performance in the State of Ohio prior to 30 years ago. (Certification requirements do no apply to posthumous candidates.)
Award for Superior Performance
This award is for those bowlers who have participated in sanctioned/certified bowling in Ohio for 15 years or more, with outstanding accomplishments in any USBC or Bowling Centers Association of Ohio certified leagues and/or tournaments. Awards will be given in 2 categories, both male and female. (Certification requirements do not apply to posthumous candidates.)
Friend of Ohio Bowling
For anyone who has supported or promoted Ohio Bowling and Ohio Bowlers for a period of at least 15 years.

We are presenting Hall of Fame certificates on-line for local associations to complete for persons being inducted into local Halls of Fame. Simply fill in the name of recipient and local association and print.
